Population Health Management
CCM was one of the first organizations to develop a Population Health program for the self-funded market in 2008. We sought validation of our program and results early by attaining accreditation in Population Health. Today, we leverage claims data to engage your entire population through member specific campaigns, whether they are high cost, low cost, high risk or low risk members.
Data Analytics
Data analytics helps us to identify critical gaps in care. Using risk indicators and predictive modeling analytics, CCM detects emerging high cost cases years before a high cost claim is ever filed.
Predictive Risk Analytics
Using Predictive Risk Analytics, CCM is able to calculate a forward looking risk score for every member within the population. Based on member specific criteria such as co-morbidity, disease burden, use or non-use of medical services or prescription medications and patterns of treatment, CCM evaluates specific risks including likelihood of hospitalization (LOH), likelihood of emergency department use (LOED), and future cost of care. This allows CCM to implement intentional campaigns targeting actionable risks in specific populations.
Early Cancer Detection
Regular cancer screenings save lives. Using data analytics, CCM is able to identify members overdue for cancer screenings. CCM provides education and encouragement for members to seek gender and age appropriate cancer screenings.
Member Engagement
CCM targets members with the most prevalent disease states and care gaps to promote compliance with evidence-based care guidelines. With an opt-out rate of less than 1%, CCM’s population health program is highly successful in engaging members in their personal health. One of CCM’s programs encourages regular visits with primary care providers, preventing complications and avoiding costly hospitalizations and emergency room visits.
Member Access to Chronic Care Professional Nurse
CCM provides members direct dial access to a Registered Nurse who is a certified Chronic Care Professional. CCM’s certified Chronic Care Professionals encourage self-care and patient engagement to support better treatment adherence. The nurse is available to answer members’ questions regarding their journey through the healthcare system by phone or e-mail.
