Population Health FAQ’s

Who is Corporate Care Management (CCM)?

CCM is an independent company focused on prevention, disease management
and case management. We follow strict national standards for health care and
data privacy. We are working with your health plan to help improve its members
overall health. This program is being provided by your employer or plan at no
charge to you.

What is the goal of the Program?

CCM reaches out to employees and dependents who are active health plan members to improve overall health. We provide information and reminders to help you take control of your health needs.

We focus on sending the right message to the right person, at the right time.

How do I enroll in the Population Health?

There is no need to enroll. You are already included if you are a member of the company’s health plan.

What can I expect?

First, you will receive a welcome letter in the mail with information about the program. Throughout the year you may receive messages that give you information about health topics, tests and screenings that may be important to you. You may also receive a call from a nurse to urge you to see your doctor.

How can I reach a Population Health Nurse*?

You can reach a nurse weekdays from 9am to 4pm. All messages are treated as
confidential.

*This is a preventative service; always call 911 if you are having a
medical emergency.

Is my participation voluntary?

Yes! If you do not want to take part in this program, simply email or call us (see above). If you choose to opt out, it will not affect your benefits. You may opt out at any time. However, we hope you will take advantage of this program.

Can you tell me if your suggested screenings are covered?

In most cases screenings and tests we remind you about are covered. Please call the number on the back of your health insurance card for specific questions about coverage.

How does CCM know what screenings and tests I am missing?

We receive secure and confidential medical claims data to tell us what screenings
and lab tests you may be missing. Messages may be sent to you if we notice that
you are missing claims for certain cancer screenings or laboratory tests known to
be standards of care for your age, gender or for certain chronic conditions you
may have.

Who does CCM share my health information with?

We may report to the company that pays your claims or to your employer. To
guard your privacy, when we report to employer, we refer to your case by a
number which we create. This number is unique to you. CCM complies with
HIPAA regulations.

What if my doctor doesn’t agree with the screenings or tests you tell me about?

Decisions about your medical care are personal decisions between you and your doctor. Your doctor is the best judge of which screenings and tests are right for you. Our letters should not be considered as medical advice. Our reminders are based on nationally recognized standards of care.

The nurse I had is an asset to the Corporate Care Management team. She is an efficient communicator, expert in resolving and identifying management of our daughter’s illness and a skilled listener.

 

I’d like to say my CCM nurse was very helpful in getting a STAT MRI approved for me, and this was the fastest approval I have ever gotten.

 

I have had the “sugar” for 10 years, my Dad had it and my son has it. I like having a nurse available to me so that I am able to call her if I have questions regarding my health. Her name and number is taped to my refrigerator. It’s a good benefit to have.

 

I handle all of the authorizations for a large practice and I will say that I work with many different authorization vendors, and there are none more professional than all of your associates who work at Corporate Care Management that I’ve had the pleasure of speaking with.

 

This program is valuable and the nurse was helpful. I can understand the importance of following up with my diabetes.

 

My representative was wonderful. I didn’t even know this service existed. It’s nice to know when you’re recovering from surgery and unable to make all of the calls needed, someone can be your advocate. The nurse took all of the worry from my shoulders.

The nurse that has been calling us is the most wonderful, encouraging person that I think I have ever met in my 70 years. She has been a friend to our family. Not just advising but a friend.
