Population Case Manager (RNs)

Corporate Care Management, Inc. (CCM) is seeking a Population Case Manager.  Using evidence based clinical tools and medical data review, our Case Managers evaluate medical service utilization, assess patient’s needs, and apply clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers to care.  This wholistic approach helps manage complex health conditions and address social indicators related to acute and chronic conditions of a population.

This is a telephonic Population Case Manager role.  Our open and inviting office space, overlooking an executive 9-hole golf course, encourages creativity and collaboration.  CCM’s hours of operation are 8am to 4pm weekdays.  No nights, holidays or weekends!

Requirements:

  • An active, unrestricted RN license (new graduates will be considered)
  • Expert clinical knowledge
  • Understanding wholistic approach to patient care
  • Passion for helping others regain or maintain their health
  • Willingness to think outside the box
  • Excellent communication, computer, interpersonal, organization and team building skills
  • Availability Monday through Friday, 8am to 4pm

Professional Nurses (RNs)

At CCM, our Professional Nurses thrive in a pleasant and collegial environment where they can continue to help patients and their families without the strains of direct patient care.

If you have:

  • A drive to learn and explore a new role of nursing, interested in expanding your knowledge domains in case management and disease management,
  • An active unrestricted NYS Professional Nursing License,
  • At least two years recent experience working in the acute care setting or in direct patient care,
  • Excellent communication and keyboarding skills, and
  • You are ready to experience a schedule that respects your family and home life, we would like to hear from you!

Care Coordinator (LPNs)

The LPN Care Coordinator assists injured or disabled workers in coordinating and gaining access to needed treatment and testing in an effort to return them to the workforce as soon as they are medically able. Previous care coordination or case management experience preferred.

If you have:

  • A drive to learn and explore a new role of nursing, interested in expanding your knowledge in care coordination,
  • An active unrestricted NYS LPN License,
  • A minimum of 2 years direct patient care experience,
  • Excellent communication and keyboarding skills, and
  • You are ready to experience a schedule that respects your family and home life, we would like to hear from you!

I enjoy being able to speak with your Registered Nurse about my progress with Diabetes and my frustrations.

 

I have had the “sugar” for 10 years, my Dad had it and my son has it. I like having a nurse available to me so that I am able to call her if I have questions regarding my health. Her name and number is taped to my refrigerator. It’s a good benefit to have.

 

This was my first experience with a Workers’ Comp situation.  The coordination and communication on your part was excellent.  [My CCM Nurse] was an excellent source of information and very helpful while I was out of work and after returning to work.  I was able to rely on her for any follow up needed.  She also offered to forward emails as I was not able to logon to my computer (went ABOVE and beyond expectations).

 

I work frequently (nearly every day of the week) with the representatives of CCM and it is always a great, easy, and quick call. They are always friendly and help with anything I request, even when it is multiple patients. Job well done, they are fantastic!

 

This program is valuable and the nurse was helpful. I can understand the importance of following up with my diabetes.

 

My CCM nurse was amazing helping me. She responded in a professional and timely fashion.  Every time I spoke with her she was very pleasant and helpful with every question.

 

This is a good program. It’s nice that the nurse calls to see how I am doing. She answered some good questions for me. I am now retired and I was a welder. Having my insurance and being able to speak with a nurse is a good benefit for retirees.

 

I handle all of the authorizations for a large practice and I will say that I work with many different authorization vendors, and there are none more professional than all of your associates who work at Corporate Care Management that I’ve had the pleasure of speaking with.

 

The CCM Intake Coordinator was a great pleasure to work with.  She was quick and efficient and held a decent conversation with me as we worked through getting prior authorization.  She is a great individual to work with.

 

My CCM nurse has gone above and beyond the call of duty, took her time to go over all issues with me and resolved all problems.
