About CCM

We started 30 years ago as a nurse rehabilitation company serving Workers' Compensation and No-fault insurance carriers.

Our mission was to save money spent on the medical and indemnity costs, through aggressive case management.

These simple beginnings have allowed us to expand our services to self-funded benefit plans, employers, third party administrators, and attorneys.

CCM nurses are trained in both the medical and business aspects of case management.

A nurse consultant is assigned to each indemnity case and your employee is guided through the health care system each step of the way.

Injury Assessment

 

We start by conducting an Injury Assessment to give the adjuster, employer and CCM's case manager a shared understanding of the case.

We contact the employee and provide a summary report the next day so you know where you stand. 

  • An injury assessment provides:
  • Claimant's description of the injury,
  • Medical treatment to date,
  • Listing of pre-existing medical conditions,
  • Physicians who have treated the claimant for pre-existing conditions,
  • Previous work-related injuries and illnesses, and
  • Recommendations for Employer, TPA, and Case Management.

We also outline hindering factors that may delay the employee's eventual return to work or prolong the healing period.

The injury assessment includes narrative impressions and recommendations from the nurse so you have a clear understanding of the case and what lies ahead.

CCM's injury assessment report was developed over 30 years of case management, with input from employers and claims adjusters.

The Injury Assessment forms the basis for case and claims management that follows.

15-8 or Second Injury fund cases are quickly identified and approvals can be granted immediately for services still pending authorization.
